Medical Expense Calculator | Illustrative Budget

Illustrate one person's annual premiums and shared-pool deductible/coinsurance costs using covered in-network allowed charges, without copays or coverage decisions.

U.S. illustration for one person and a new plan year, with no prior deductible or out-of-pocket spending. Prescription and other inputs are covered in-network allowed charges before cost sharing, not billed charges, copays or costs already paid by you. Both categories are assumed subject to the same deductible, patient coinsurance and out-of-pocket cap. Use amounts from your actual plan's allowed-rate information; no prices are supplied here. Copays, separate drug deductibles, family rules, preventive exemptions, noncovered/out-of-network care and balance bills are excluded. This model does not determine coverage, eligibility or tax-qualified expenses.

The combined allowed charges pay the deductible first; the entered patient coinsurance percentage applies to the remainder and rounds once, half up to cents. Modeled care costs are capped, while 12 monthly premiums remain outside that cap. The remaining deductible is the modeled unused amount after these assumed charges, not an insurer's current accumulator. The supported shared-pool scenario requires deductible ≤ cap; equality, including zero/zero, is valid. Zero amounts are hypothetical inputs, not proof of free coverage. All money inputs allow cents and percentages allow hundredths; invalid or inconsistent entries hide results and disable export. Plan conditions, assistance and actual claim processing can change costs. The entered cap is not a maximum for your entire budget or excluded expenses.

Build a modeled annual medical budget

U.S. illustration for one person and a new plan year, with no prior deductible or out-of-pocket spending.

This U.S. illustration models one person's new plan year, starting without any deductible or out-of-pocket amounts already paid. Enter prescription allowed charges and other covered in-network allowed charges before patient cost sharing. The allowed amount is the amount recognized by the plan, not necessarily the provider's bill. A prescription receipt showing your copay or a medical bill showing what you owe is not the same input. The two categories are added together and use one deductible and one patient coinsurance percentage, then one out-of-pocket cap. They are not separate drug and medical benefits.

Understand one shared cost-sharing pool

The annual budget combines twelve monthly premiums and modeled patient care costs. Premiums remain payable under the entered assumption even when allowed charges are zero, and do not count toward the care-cost cap. For combined allowed charges, the model applies the deductible first. It applies your patient coinsurance percentage only to the excess, then limits total patient care spending to the entered cap. The remaining deductible reports the unused portion of this single modeled deductible; it is not the insurer's actual accumulator balance. No copays are calculated, and the model does not decide whether a particular medication or procedure is covered.

Compare the assumptions with your plan documents

Use the result to make a budget scenario explicit, then compare it with the actual Summary of Benefits and Coverage and full plan documents. Check which prescriptions and services share the deductible, whether any have fixed copays, and which spending counts toward the cap. If your plan has a separate drug deductible, a family deductible or service-specific rules, this model may not represent it. Revisit the entered allowed amounts when more information is available. The annual figure is not a promise about the largest possible medical bill and does not establish tax deductions, HSA or FSA eligibility, enrollment eligibility or claim payment. A PDF preserves the illustration and its assumptions, not an insurer determination.

How to use this calculator

  1. Enter a monthly premium, annual deductible and out-of-pocket limit in U.S. dollars with at most two decimal places. This supported one-pool scenario requires the deductible to be no greater than the entered limit.
  2. Enter the patient's coinsurance percentage from 0 to 100, with at most two decimal places. This is the patient's share after the deductible, not the insurer's percentage and not a tax rate.
  3. Enter annual prescription and other covered in-network allowed charges before cost sharing. Keep the categories separate without counting the same charge twice. Do not enter billed charges, copays or amounts already paid by the patient.
  4. Review annual premiums, patient care costs, total annual budget and modeled remaining deductible. Compare the chart's two components with the total. Zero is valid; fix blank, negative, excess-precision or unsupported entries before results and PDF export are available.
